We will contact you once registration is open.
Children's Paperwork
Once you have confirmed your child's space in our program, please follow the links below to fill out their enrollment paperwork.
Please note that all forms must be printed and returned to The Dolphin Club with original signatures.
Additionally, we have one form that must be filled out in person at our facility.

Family Handbook
The Dolphin Club Family Handbook is available in hard copy at The Dolphin Club or by asking the Program Director for a copy.

If your child requires medication that is to be given at Dolphin Club, a Medication Consent Form will be required.
